The Falcons, pending confirmation of league status for next season will play in Pool 5 of the tournament.
They will find out if they are to be relegated on June 21 after London Welsh' appeal for promotion to the Aviva Premiership.
The draw throws up a rematch of the 2004 Heineken Cup quarterfinal against Stade Francais.
The first game is scheduled for October 11 and the final pool match is on January 17 2013.
The Falcons’ game against Grenoble will see new director of rugby Dean Richards face a former club.
Richards coached the recently promoted French side between 2004 and 2005.
The Falcons have never played Italian team I Cavalieri Prato.
They finished second in ‘Classifica Eccellenza’ last season.
